6- A New Form of Dressing After Hypospadias Surgery

Introduction:

Hypospadias operation is among common surgical operation practicing in the Division of Pediatric Surgery of Children’s Hospital, Bandar Abbas, Iran. Several kinds of post-operation dressing are currently available with their benefits and side effects. We are, hereby, presenting a new method of dressing using cyanoacrylate (CA) glue in these patients. Generally, cyanoacrylate is an acrylic resin which rapidly polymerises in the presence of water, forming long, strong chains, joining the bonded surfaces together.

Patients and Methods:

Sixty-one patients with stage II hypospadias and a mean age of 13.5 month, were randomly enrolled in this prospective study. In 45 patients after operation, conventional dressing was applied while in 16 patients, cyanoacrylate (CA) glue was used for dressing. After completion of operation, while the penis was held in erect position using a silk suture line, CA applied around the shaft of penis and its base in a total of 4 times with an interval of 2 minutes waiting to be dried. All patients were followed postoperatively for at least 2 weeks. Side effects were compared.

Results:

In conventional dressing, 12 patients were complicated by infection, hematoma, painful removal of dressing, and need for repeated dressing. The only one hematoma in CA group was easily diagnosed through transparent CA and rapidly managed. No patients needed repeated dressing or chane of dressing to conventional forms.

Conclusion:

CA is impermeable to urine and stool, transparent, less painful with a comparable price. We suggest this synthetic glue for hypospadias postoperative dressing.
